1. What data we collect
On our website you can request a consultation by leaving your name and phone number. Together with the request we store the date and time it was sent and the path of the page it was sent from. This data is used to get back to you and process your request.
2. Why we collect it
We collect contact details to get in touch with you, answer your questions and schedule a consultation. We do not use your data for marketing without your explicit consent.
3. Storage and protection
Requests are stored in Google Sheets with access limited to the administrator and the doctor. A notification with your name, phone number, date and page is sent to the clinic's private Telegram group. Google and Telegram are used as services to store and transmit this data. To protect the form from automated submissions we use Google reCAPTCHA, which processes technical data about your interaction with the site. We do not publish your contact details or use them for marketing without your consent.
4. Your rights
You have the right to request deletion or correction of your personal data at any time by emailing dr.prykhodko.danylo@gmail.com.
5. Cookies and analytics
The site uses technical browser storage needed for it to work, including to remember your chosen language and your cookie decision. Analytics cookies are enabled only after you click “Accept” in the banner: Google Analytics (Google) counts visits and traffic sources, and Microsoft Clarity (Microsoft) records anonymised interaction with pages — clicks, scrolling, cursor movement — so we can see what is inconvenient on the site. This data is processed on Google and Microsoft servers, including outside Ukraine. We do not use marketing or advertising cookies. You can change your decision at any time via the “Cookie settings” link at the bottom of the page.
